Problem updating to to 4.2 thru OTA Android Robot exclamation

I do not know what is the problem of my device but I have encountered this issue and still
persistent. I have unlocked Galaxy Nexus bought from Play store. I got the notification that the
4.2 is ready to install but this what happened to me during the installation process, i got the
android robot with the exclamation mark. I reset my device and wiped everything and try to
install the OTA and I still get the same error. Is there a problem with my GN ?
